Life Experiences & Patterns Assessment

For each item, consider how often the statement reflects your experience and select the response that best fits. Use the scale provided in the drop down, where:


Never means the statement does not apply to you at all

Rarely means it happens infrequently

Sometimes means it occurs on occasion

Often means it happens regularly

Very Often means it is a consistent or frequent experience


Try to answer based on your typical patterns rather than focusing on isolated situations.

PART 1: Life Experiences
